Before filling out a registration form, check you are in our catchment area.
We accept patients living in Beverley and the surrounding villages within a 3 mile radius, if outside of this please contact the surgery to check our practice boundary.

Registering with the surgery
To register with the surgery:
- fill out an online registration form
- visit reception during our opening hours and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
You will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Medical treatment is available from the date of registration. Please contact reception for further information.
New patients with regular medication are advised to make a telephone appointment with the practice pharmacist, more complex medication may require a GP appointment.
Named GP
All patients will be allocated a named GP who is responsible for that patient’s overall care at the practice. If a patient has a preference as to which GP that it is, the practice will make reasonable efforts to accommodate this request.
Patient Behaviour and Contact Contract
The practice will review your existing medical record and if deemed necessary issue a patient behaviour and contact contract to ensure boundaries are adhered to.
